Contraindications:

Procedures:

Proper fitting requires trying multiple sizes to determine the correct pessary size. The Pessary Fitting Set is an essential tool for accurate selection. Physicians may recommend estrogen therapy and/or lubricants before insertion to enhance comfort and ease of placement.

  • Perform a normal pelvic examination prior to the fitting and introduction of a pessary. A pelvic exam helps determine the appropriate size. The pessary will easily fold across the largest drainage holes to simplify insertion and removal. The pessary should be large enough for its indication, yet not cause any undue pressure or discomfort.
  • Compress the pessary and gently insert the pessary through the introitus. Once the pessary has passed the introitus, release the compressed pessary so that it expands to its normal shape. Each type has a similar method of insertion; however, some types have special methods of insertion into the vagina.
  • For the Cube Pessary, ensure the loop faces outward during insertion.
  • Gently move the pessary to the cervix to support the cystocele or rectocele and the uterus. When in the correct position, the physician should be able to insert an examining finger between the outer edge of the pessary and the vaginal wall. This spacing will ensure the patient’s comfort and reduce the risk of pressure necrosis. All types are placed in a similar position.
  • Once in place, the pessary should not dislodge when standing, sitting, squatting or bearing down. It should not be uncomfortable for the patient during any of these routine activities. Pessary should be large enough for its indication, yet not cause any undue pressure or discomfort.
  • The prescribed pessary should be cleaned prior to each insertion and removal. The cleaning steps are especially recommended before your first follow-up visit.
  • The Patient should report any discomfort to their doctor immediately.

Removal and Suggested Follow-Up:

  • To remove, locate the pessary loop using the index finger or a suitable tool and gently pull downward through the introitus.
  • Clean the pessary every one to two days to maintain hygiene and comfort.
  • Some users remove their pessary during sexual activity; consult your physician if unsure.
  • Discuss long-term use, replacement intervals, and follow-up schedules with your healthcare provider.
  • Monitor for irritation, discharge, or sensitivity within 24-48 hours of initial use and report any concerns promptly.
  • Routine follow-up visits help ensure continued comfort, proper fit, and optimal performance.

Cleaning:

  • Before initial use, wash the pessary thoroughly with warm water.
  • Rinse with warm water (20-40°C) for 30-40 seconds.
  • Use a mild pH 5-9 soap and gently rub the pessary about 10 times to form a good lather.
  • Rinse with warm water for one minute; repeat if necessary.
  • Perform a final rinse with distilled water for one minute to remove soap residue completely.
  • Allow the pessary to air-dry completely before reinsertion.

Caution:

  • Do not use the pessary if any visible defect, such as cracks or discoloration, is present.

What is a Pessary?

The pessary is a vaginal prosthesis that has evolved over centuries to become an effective medical device to relieve the stress of uterine or bladder prolapse, cystocele, rectocele, and often improves the symptoms of urinary incontinence.

Pessaries are distributed in a variety of styles, shapes, and sizes. They are perfectly safe, effective, and comfortable for long-term usage.

A pessary is a mechanical device, not unlike the common diaphragm, made of non-toxic medical-grade silicone and used to support the lower pelvis. This material is biologically inert, so allergic reactions are rare. Pessaries can be autoclaved (except donuts) or boiled or sterilized using cold sterilizing agents (all pessaries).

What is Genital Prolapse?

Genital prolapse is a general term referring to several conditions that may occur separately or in combination. These include a uterine prolapse (dropped womb), a vaginal prolapse, a cystocele (dropped bladder), a rectocele (dropped rectum), and an entrocele (herniation of the small intestine into the space between the rectum and vagina).

Causes of a Prolapsed Uterus?

A uterine prolapse occurs when the uterus drops from its normal position. The cervix may descend into the vaginal opening. This condition affects approximately 11% of all women, usually in the post menopause years. However, it also occurs in young women, and it may be caused by the childbirth of a large baby when the vaginal walls are damaged.

Multiple deliveries may also weaken the area and lead to a uterine prolapse. Other causes include constipation, obesity, and fibroid tumors. Sometimes in menopause, the ligaments that hold the uterus in place lose some elasticity and strength due to reduced estrogen levels, and this may lead to a uterine prolapse.

Symptoms of a Prolapsed Uterus?

The symptoms of uterine prolapse usually include a feeling of pelvic bulging, heaviness or fullness, and/or lower back pain. These symptoms may worsen with activity and improve with bed rest. When standing, women with a prolapse actually feel as though something is falling out. Other symptoms may include vaginal pain or discharge, incontinence or difficulty urinating, bladder infections, or anal pain.

Pessaries are used to alleviate these symptoms. Not only do pessaries assist in restoring continence by stabilizing the bladder base, but they can also support the pelvic organs in their proper anatomic position in women who have a prolapse, cystocele, or rectocele. In women who are not interested in surgery or in women who are not good surgical candidates, pessaries offer a very viable treatment option.

Will I always have to use this pessary?

Not necessarily. Your vaginal muscles may strengthen to the extent that the pessary is no longer needed. Sometimes pessaries are only required for strenuous activity.

Can a pessary hurt or become uncomfortable?

A properly fitted pessary never causes pain or discomfort.

How often should I remove the pessary?

It is recommended that you remove the pessary daily to clean it using a mild soap and water. If you have difficulty removing it, make an appointment with your healthcare provider for removal and cleaning. Don’t be alarmed if the appointment is weeks away. Pessaries can remain in place from 4 to 12 weeks.

What is my responsibility?

Will my pessary set off the alarm at the airport?
